• Foreign citizens must provide proof of citizenship, such as a birth certificate, consular birth certificate, passport, certificate of entry into foreign citizenship or a certificate of citizenship.
• If you are not a US citizen, you must provide proof of immigration status and rights to work in a given country.

Apply for a replacement card in person at your local social security office. Take with you the completed application form and all the necessary documents.

You will receive a new card in the mail within 2 weeks.

How to replace your child’s lost social security card

Be prepared to provide proof of the identity of your child and yourself.

• Documents accepted as evidence of the child’s identity include one of the following originals: school identification card, information about the school or educational institution, decision on adoption, medical or religious documents (for example, baptism).
• Documents accepted as proof of your identity are the same as above.

Collect other required original documents that you must provide if your child is a foreign citizen. US citizens born in this country may not submit additional documents.

• If your child was born in the USA but is a citizen of another country, you must provide one of the following documents: birth certificate, consular birth certificate, passport, certificate of foreign citizenship or certificate of citizenship.
• If your child is not a US citizen, you must provide a form about arrival and departure from the country of l-94, which your child will receive upon arrival in the United States.
• If your child came to the United States by exchange, you must show the DS-2019 form, a certificate of visitor status for the exchange.
• International students must show the l-20 form, student certificate of nonimmigrant status.

Apply for a replacement card in person at your local social security office. Take with you the completed application form and all the necessary documents.

You will receive a new card in the mail within 2 weeks.
